When you schedule your first appointment at Envision Ophthalmology & Wellness, all new patients will be emailed a secure link to complete new patient forms online prior to your visit. This helps to make the process easier for you and increases accuracy and efficiency. This information then goes directly into your Electronic Health Record (EHR) at Envision.
For those patients without an email address or who are unable to access the online forms through the secure link, we have made printable copies available for download here. Please print, complete and bring them with you to your first appointment.
